Day Centre for Dementia

Since February 2020, witin the framework of the Operational Program “WESTERN MACEDONIA” {priority axis: 10 “Advocacy of Social Integration and Poverty and Discrimination Alleviation ”}, WITH THE TITLE “ACTIONS PROVIDING HEALTH SERVICES/ACTIONS SUPPORTING MENTAL HEALTH” which is co-financed by the European Social Fund (ESF), with code ΟΠΣ 5044930, the Day Centre for Dementia has started operating at B’7 Philipou Street, Kozani, providing specialised services to people diagnosed with dementia as well as to families-caregivers of the patients.

All services by the Day Centre for Dementia are provided completely free of charge in the region of Western Macedonia, covering the needs of its districts (KOZANI-GREVENA-KASTORIA-FLORINA) and their bordering districts and municipalities.

The purpose of the Day Centre for Dementia is the organisation and application of group and individual therapy programs for the people diagnosed with Alzheimer disease or other forms of dementia, who are in the early, middle or later stages of the illness.

The staff at the Day Centre for Dementia in Kozani, is comprised of expert healthcare professionals of various specialisations:

  • Psychiatrist
  • Psychologists
  • Social Worker
  • Speech Therapist
  • Physical Therapist
  • Nursing staff

More specifically, the services provided, completely free of charge by each specialisation are as follows:

 

  • Evaluation of cognitive and language skills.
  • Group or individual workshops on language and speech intervention therapy.
  • Vocabulary and language exercises such as sentence completion and comprehension, hearing comprehension, crossword puzzles, etc.
  • Cognitive skill enhancement exercises (memory, focus, attention), word recall, patterns, similarities, etc.
  • Fine movement and visuospatial exercises, such as mazes, drawing and writing activities.
  • Face muscle exercises (individual).
  • Breathing exercises, voice and enunciation strengthening.
  • Hand movement exercises (drawing, writing)
  • Diagnosis and treatment of dementia.
  • Placement of attendees into groups aiming to improve their cognitive functions.
  • Tracing the level of cognitive deficiencies after scanning the assessment scale of higher brain functions (memory, judgement, subtractive ability).
  • Carrying out of research.
  • Scheduling informational seminars to bring awareness to the community over the consequences and treatment of dementia.
  • Support - Consultative psychotherapy for caregivers.
  • Promoting the utility of the Day Centre for Dementia to the community.
  • Administration of pharmacologicaltreatment.
  • Evaluation of cognitive skills.
  • Cognitive training for the elderly with dementia with high, medial and low functionality, aiming to preserve their cognitive skills.
  • Cognitive training for the elderly with mild cognitive disorders, aiming to prevent and stabilise cognitive skills.
  • Consultative support for caregivers of the elderly diagnosed with dementia, aiming to psychoeducate the cares on issues on dementia and to offer support throughout the illness.
  • Designing and implementing prevention and awareness programs pertaining to issues regarding the elderly (experiential actions in associations and Open centres for the protection of the elderly, local communities of Western Macedonia)
  • Designing and implementing networking and collaborative programs with sectors of Western Macedonia (Open centres for the protection of the elderly, Library, Local Authorities).
  • Training of staff and caregivers in contact with the elderly, aiming to provide skills necessary to managing the illness.
  • Emotional management groups for the elderly with high functionality or mild cognitive deficiencies, aiming towards consultative support and social enhancement.
  • Providing social services and collecting the social histories from the caregivers of the patients with information on the large scale of social welfare and legal issues regarding the elderly diagnosed with dementia, for the facilitation of bureaucratic procedures.
  • Social support for the caregivers/relatives of the patients who are facing difficulties.
  • Networking with other community sectors in collaboration with the Liaison.
  • Επικουρική συμμετοχή στις ομάδες εργοθεραπείας.
  • Archiving of electronic files of the patients.
  • Physical therapy services aiming to reduce pain, improve mobility, maintain good muscle functionality and body awareness.
  • The physical therapy program is implemented based on rehabilitation protocols, individualised and adjusted to the needs of each patient after evaluation and always in conjunction with and under the guidance of the Association's Medical professional.
  • There are also group consultations of therapy exercises (3- 4 people), aiming to maintain the range of joint movements, strength and muscle flexibility, as well as the improvement of respiratory and cardiovascular functions, while promoting community spirit and collaboration.

With an anthropocentric approach and taking into account the dignity and respect of the patient’s character, there is a daily routine of: taking the patient’s vitals, administering pharmacological treatment if it is necessary to be given at specific times during which the patient is in our presence.

Covid-19 certificate inspection.
Attending to the hygiene of the association’s spaces, as well as to the required medical supplies.
Attending to the patient’s transportation within the association’s spaces and overseeing the groups of occupational therapy.
Informing caregivers on the most appropriate care process towards the patients.

At the Day Centre for Dementia the following programs are implemented:

Στο πλαίσιο του ΕΠΙΧΕΙΡΗΣΙΑΚΟΥ ΠΡΟΓΡΑΜΜΑTΟΣ «ΔΥΤΙΚΗ ΜΑΚΕΔΟΝΙΑ» {άξονας προτεραιότητας: 10 « Προώθηση της κοινωνικής ένταξης και καταπολέμηση της φτώχειας και κάθε διάκρισης (ΕΚΤ)»}, ΜΕ ΤΙΤΛΟ «ΔΡΑΣΕΙΣ ΠΑΡΟΧΗΣ ΥΠΗΡΕΣΙΩΝ ΥΓΕΙΑΣ/ΔΡΑΣΕΙΣ ΣΤΗΡΙΞΗΣ ΨΥΧΙΚΗΣ ΥΓΕΙΑΣ», το οποίο συγχρηματοδείται από το Ευρωπαικό Κοινωνικό Ταμείο (Ε.Κ.Τ), πραγματοποιείται το πρόγραμμα ΔΙΑΣΥΝΔΕΣΗΣ ΤΟΥ ΚΕΝΤΡΟΥ ΗΜΕΡΑΣ ΓΙΑ ΠΑΣΧΟΝΤΕΣ ΑΠΟ ΑΝΟΙΑ-ALZHEIMER, ΜΕ ΜΟΝΑΔΕΣ Ο.Τ.Α. (Κ.Α.Π.Η.-Κ.Η.Φ.Η.), με ΚΩΔ.ΟΠΣ 5045474, στην Κοζάνη.

For the implementation of the Liaison of mental health services and developing dementia and Alzheimer friendly spaces in the wider area of Kozani and the bordering districts, partnership memorandums have been signed with following units of social care of elderly by Local Authorities (ΟΤΑ):
